The Spine Institute of New York provides a wide variety of patient services. Our team of spine care professionals is dedicated to serving you and ensuring that the care you receive is coordinated seamlessly.
Appointment RequestTo request an appointment at The Spine Institute of New York, please complete one of the forms to the right. Our staff will contact you within 48 hours to schedule your appointment, excluding weekends and holidays. Prescription Refills Request
The Spine Institute of New York physicians and nurse practitioners will prescribe pain medication for a limited time as part of patient's care. If ongoing pain management is necessary, patients will be referred to their primary physician or a pain management specialist.
Medications are refilled Monday through Friday during business hours (8:00am - 3:00pm) excluding holidays. Please allow 48 hours for refill requests to be processed. Medications are not refilled on evenings or weekends.
Clinical QuestionsThe Spine Institute of New York physicians and nurse practitioners will answer questions submitted online for existing patients regarding their medical condition and ongoing treatment. Patients who have not been seen at the Spine Institute should NOT submit questions online. New Patients are advised to Request an Appointment. Medical Records RequestSpine Institute of New York maintains compliance with HIPAA regulations regarding confidentiality of patient's medical records. A signed Release of Medical Authorization Form is required from the patient to obtain copies of medical records. There is a fee associated with the copying of medical records.
