Initial Visit

For us to best treat your spinal condition or injury, we need to have a comprehensive understanding of your current health status. When you come in for your initial consult, please bring all of the items listed in this article, as well have the forms mentioned, printed and completed prior to your visit.

 

Pain Management Injections

Please follow the instructions in these articles if you are scheduled to have an Epidural Steroid Injections or Spine Joint Injections.  They contain information regarding what to expect before, during and after your procedure.

 

Surgery Instructions

Please follow the instructions in these articles if you are scheduled to have spinal surgery. They contain information regarding what to expect before, during and after your surgery.  The articles also provide useful addresses and other helpful contact information.
